Medical Record Review & Chronologies

Medical record review is the foundation of every medical-legal matter. The question is not whether the records were read. It is whether they were read by someone who understands how the file will be used once it reaches litigation.

Lex Fidelis produces timestamped, citation-anchored medical chronologies built directly from the source record. Every entry is tied to the document it came from. Documentation gaps, inconsistent entries, and evidentiary weaknesses are identified and labeled, not omitted because they are inconvenient. The deliverable is structured for deposition preparation, expert briefing, and trial narrative from the first draft.

Mass Tort & Litigation Support

Mass tort litigation support is only as valuable as the analytical framework behind it. Senior clinical review, structured causation analysis, and attorney-ready work product are a function of the standard applied to every engagement, not the specific docket.

Lex Fidelis supports mass tort and complex litigation matters, including plaintiff screening and stratification, causation analysis, exposure history documentation, large portfolio record review, and damages support. The founding principals bring direct, active experience across toxic exposure, pharmaceutical, medical device, and environmental dockets. Attorneys bringing matters outside that portfolio receive the same senior principal review and the same analytical standard.

Standard of care analysis

A standard-of-care analysis is not a summary of what happened. It is an assessment of whether what happened met the accepted standard of practice and whether that determination will hold up when challenged.

Lex Fidelis conducts standard-of-care analyses for medical malpractice, personal injury, and institutional liability matters across a broad range of clinical practice settings and care environments. Analysis is anchored to the medical record, applicable clinical standards, and the specific practice setting at issue. Deviations are documented with specificity. Where the record is ambiguous or silent, that limitation is stated, not papered over.

The standard of care analysis conducted by principals holding a BSN, RN, and JD, alongside a BSN, RN, and CLCP, is built to withstand deposition, expert challenge, and cross-examination. The combined credential set means the analysis is structured with both legal precision and damages context from the first page. Life Care Planning & Medical Costs Assessment

Lex Fidelis produces life care plans for catastrophic personal injury, occupational injury, birth and pediatric injury, medical malpractice damages, and long-term care matters.


Life care planning is among the most technically demanding services in medical-legal consulting. A defensible plan requires disciplined analysis of the medical record, consistency with established standards of care, and clear linkage between injury, treatment needs, and projected costs. Opinions must be methodologically sound and able to withstand scrutiny in deposition and at trial.

Utilization review

Utilization review is key to determining whether the medical care provided, requested, or denied was truly necessary for a patient’s condition based on accepted medical guidelines. How useful that decision is depends on whether the reviewer understands both the medical standards being applied and how the decision will be used—whether for patient care, insurance coverage, or legal purposes.

II. LABEL ALL ASSUMPTIONS

When inference is required, it is identified as such. Ambiguity is flagged, not silently resolved in favor of the client’s preferred narrative.

III. IDENTIFY MISSING DATA

Gaps in the record are documented and their evidentiary significance assessed explicitly. Missing data is not filled. It is identified, and its impact on the analysis is disclosed.

IV. SEPARATE FACTS FROM ANALYSIS

Every deliverable distinguishes between what happened, what it means clinically, and what remains uncertain. Facts are not presented as conclusions, and conclusions are not embedded in narrative summaries where they cannot be tested.
